Keywords: Active surveillance; Kidney cancer; Watchful waiting EN Active surveillance Kidney cancer Watchful waiting 2825 2829 5 09/01/21 20210801 NES 210801 Introduction Over the last 30 years, kidney cancer has had a significant stage migration towards incidentally detected small renal masses (SRM) due to the increased availability and use of cross-sectional abdominal imaging [[1]]. Improved understanding of the host-tumor interaction, as well as an individual patient's overall life expectancy, will allow for a better appreciation of how treatment can be tailored to each patient. There were 15-33% of patients in these cohorts who progressed to treatment due to interval growth, patient preference, improvement in their medical condition allowing for surgery, or new symptoms [[19]-[21]]. Although the risk of metastatic disease is low for patients with an SRM at approximately 2-4%, metastatic RCC is considered incurable, and is an important outcome to avoid in patients with an SRM who would be willing and able to receive treatment [[11], [15]].
