THE EFFECT OF NEGATIVE HEEL HEIGHT ON INCREASED KYPHOTIC CURVATURE IN ADOLESCENTS WITH THORACIC HYPERKOFOSIA: PILOT STUDY.

- Abstract
Purpose: Hyperkyphosis defines as curvature at 50° and over. The use of positive and negative heel heights can have an effect on pelvic position, spinal curvatures and kinematic chain. The aim of this study is to investigate the immediate effect of the negative heel height on correcting the increased kyphotic curvature. Methods: Twelve adolescents diagnosed with hyperkyphosis (mean-age 13,75±2,09years) were enrolled. The kyphosis angle (with Flexicurve and Body Analysis Kapture System-BAK) and the body posture (with New York Posture Scale-NYPS) were assessed with the feet in a neutral position (A0), at 15◻(A15), at 20◻(A20), with wedges support. SPSS 20.0 program was used for statistical analysis and p<0.05 values were considered as significant. Results: Kyphosis angles with Flexicurve were 57,08±4,29 (A0); 53,25±3,67 (A15) and 52,88±4,43 (A20) calculated. The difference between the measurements was statistically significant (p=0,001) (A0-A15 p=0,002, A0-A20 p=0,002, A15-A20 p=0,363). The kyphotic angles assessed by BAK were 45.08±3.23 (A0); 41.58±2.39 (A15) and 41.42±2.54 (A20). There was a significant difference between the measurements (p=0.001) (A0-A15 p=0.002, A0-A20 p=0.003, A15-A20 p=0.719). Mean NYPS scores were 32,17±7,36 (A0); 41.00±5.97 (A15) and 39.25±6.30 (A20). There was a significant difference between the scores (p=0.001) (A0-A15 p=0,002, A0-A20 p=0,004, A15-A20 p=0,058). Conclusions: The negative heel height appears to be a method that can reduce the angle of the thoracic kyphosis by affecting the kinematic chain. The effects weren't different between 15-20 degrees. Also at 20◻B, NYPS scores were decreased. This may be due to the increase in inclination affects the foot posture to pronation.
