- Abstract
The authors describe the medical case of a man treated with valproic acid for bipolar disorder who experienced severe alcohol withdrawal despite a therapeutic serum level. The patient had a history of bipolar disorder, alcohol dependence and cannabis dependence. He was continued on valproic acid and was treated with diazepam for breakthrough withdrawal symptoms. Valproic acid was discontinued on the seventh day of stay at the hospital due to concerns over persistently elevated liver enzymes. Lithium carbonate was used for treatment. The case of the patient led to the observation that maintenance treatment with an anticonvulsant agent causes a degree of tolerance which then limits its therapeutic potential.
