Estructura genética y dinámica de la transmisión de malaria por Plasmodium falciparum en la Costa Pacífica colombiana.

- Abstract
Antecedentes: La Región Pacífica de Colombia contribuye actualmente con entre 30.000 y 40.000 casos de malaria al año, y más del 70 % de los casos de malaria por Plasmodium falciparum en el país. El estudio de la dinámica de transmisión de malaria por P. falciparum en zonas de baja transmisión y sus consecuencias en la estructura genética de las poblaciones parasitarias, es de importancia para explicar fenómenos como la aparición de fenotipos de resistencia a los antimaláricos y los patrones de conectividad entre las poblaciones. Debido a la baja diversidad genética de los parásitos, y a la relativa baja intensidad de picadura en humanos por parte de los mosquitos vectores, las infecciones en estas áreas son en su mayor proporción infecciones monoclonales con bajas tasas de recombinación efectiva. Métodos: Se analizaron 166 genomas completos secuenciados de muestras de Colombia y Ecuador. Resultados: Utilizando métodos para determinar la identidad por descendencia, encontramos 17 linajes clonales, de los cuales 6 han persistido en la región durante, por lo menos, una década. Además, se muestra evidencia del efecto de un fuerte barrido selectivo en dos genes asociados a la resistencia a la cloroquina (crt y aat1) y un barrido selectivo débil o incompleto en tres loci asociados a la resistencia a antimaláricos (dhps, dhfr y mdr1). Conclusiones: El efecto de la recombinación efectiva ocasional permite generar nuevas combinaciones multilocus de genes asociados con la resistencia a antimaláricos. El trabajo muestra, además, la conectividad en el tiempo de las poblaciones parasitarias a todo lo largo de la Costa Pacífica.
