Three estimates of the association between linear growth failure and cognitive ability.

- Abstract
Objectives To compare three estimators of association between growth stunting as measured by height-for-age Z-score and cognitive ability in children, and to examine the extent statistical adjustment for covariates is useful for removing confounding due to socio-economic status. Methods Three estimators, namely random-effects, within- and between-cluster estimators, for panel data were used to estimate the association in a survey of 1105 pairs of siblings who were assessed for anthropometry and cognition. Furthermore, a ‘combined’ model was formulated to simultaneously provide the within- and between-cluster estimates. Results Random-effects and between-cluster estimators showed strong association between linear growth and cognitive ability, even after adjustment for a range of socio-economic variables. In contrast, the within-cluster estimator showed a much more modest association: For every increase of one Z-score in linear growth, cognitive ability increased by about 0.08 standard deviation ( P < 0.001). The combined model verified that the between-cluster estimate was significantly larger than the within-cluster estimate ( P = 0.004). Conclusion Residual confounding by socio-economic situations may explain a substantial proportion of the observed association between linear growth and cognition in studies that attempt to control the confounding by means of multivariable regression analysis. The within-cluster estimator provides more convincing and modest results about the strength of association. Objectifs: Comparer trois estimateurs de l’association entre le retard de croissance, mesuré par le Z-score taille pour l’âge et la capacité cognitive des enfants, et examiner si la limite statistique de l’ajustement des covariables est utile pour éliminer l’effet dû au statut socio-économique. Méthodes: Trois estimateurs à savoir un estimateur des effets aléatoires et les estimateurs intra- et inter- groupes, pour des données de panel ont été utilisés pour estimer l’association dans une surveillance menée auprès de 1105 paires de frères et sœurs qui ont étéévalués pour l’anthropométrie et la cognition. En outre, un modèle “combiné” a étéélaboré pour fournir à la fois des estimations intra- et inter- groupes. Résultats: Les estimateurs des effets aléatoires et inter- groupes ont montré une forte association entre la croissance linéaire et la capacité cognitive, même après ajustement pour une gamme de variables socio-économiques. En revanche, l’estimateur intra groupe a révélé une association beaucoup plus modeste: pour chaque augmentation d’une valeur 1 du Z-score dans la croissance linéaire, la capacité cognitive augmentait de près de 0,08 écart-type (P < 0,001). Le modèle combiné a permis de vérifier que l’estimation inter- groupe était significativement plus large que celle intra- groupe (P = 0,004). Conclusion: Une confusion résiduelle due au statut socio-économique pourrait expliquer une proportion importante de l’association observée entre la croissance linéaire et la cognition dans les études qui tentent de corriger la confusion au moyen d’analyse de régression multivariée. L’estimateur intra-groupe procure des résultats plus convaincants et modestes sur l’importance de l’association.
