- Abstract
Objective: Psoriatic lesions of the nail plates are observed in 10-78% of patients. There is however scarcity of data on specially elaborated scales to evaluate changes of these skin appendages. The aim of the study was to evaluate, in quantitative and qualitative way, morphological changes of the nail plates in the patients hospitalized at the Department of Dermatology and Venereology of the Medical University of ŀódź because of psoriasis vulgaris. Material and methods: The scale proposed by Baran was used to evaluate nail changes in 100 psoriatic patients. The following features were estimated: pitting, transverse Beau's lines, hyperkeratosis, onycholysis, leukonychia, oil drops, trachyonychia (range I to 3 points for each feature). The scale range for one nail was from 0 to 21 points, 0-420 points for one patient (20 nails). The severity of skin lesions was assessed by PASI (Psoriasis Area and Severity Index), SAPASI (Self-Administrated Psoriasis Area and Severity Index) and BSA (Body Surface Area). Results: Nail changes were demonstrated in 85 of psoriatic patients and ranged from 0 to 226 points. Onycholysis (41 patients), pitting (37 patients) and trachyonychia (35 patients), were the most frequently observed in finger nails, whereas in toe nails - subungual hyperkeratosis (65 patients), onycholysis (52 patients) and trachyonychia (48 patients). Leukonychia (10 patients) and oil drops (21 patients) were the least frequently observed in finger nails; whereas pitting (4 patients) and oil drops (18 patients) in toe nails. Conclusions: Baran scale seems to be a useful tool in severity of nail involvement evaluation in everyday dermatological practice.
