- Abstract
A woman in her late 50s presented with painful lesions on her forearms that rapidly progressed and ulcerated. She also experienced fever, cough, myalgia, and hearing loss. After further investigation, a diagnosis of granulomatosis with polyangiitis (GPA) was made based on clinical symptoms, histopathological findings, and positive proteinase 3 antineutrophil cytoplasmic antibodies (ANCA). GPA is a necrotizing vasculitis that affects multiple organ systems. Treatment typically involves immunosuppressive therapy. Other potential diagnoses, such as pyoderma gangrenosum and mycobacterial infections, were ruled out.
