Zusammenfassung Die umgehende operative Behandlung einer hüftnahen Femurfraktur mit dem Ziel frühzeitiger voller Belastbarkeit gilt heute als Selbstverständlichkeit. Vergessen sind die Zeiten, da ein Patient mit solcher Fraktur monatelang immobilisiert wurde und diese Phase mit hoher Mortalität einherging. In diesem Beitrag soll ein Rück- und Überblick auf die 430 Jahre gegeben werden, seit Paré erstmals im Jahre 1575 eine solche Fraktur beschrieb. Erst 150 Jahre später erkannte man die unterschiedliche Lokalisation der Brüche, wobei die mediale Schenkelhalsfraktur (SHF) lange Zeit als unheilbar galt. Über eingestauchte mediale SH–Frakturen wurde erst 250 Jahre nach der Erstbeschreibung berichtet! Es wird besonders auf die sich langsam entwickelnde Therapie eingegangen: die Reposition, die Reduktion, nämlich die Drehung des außenrotierten Beines um die Längsachse nach innen, die Lagerung, besonders auf der doppelt geneigten Schiene, den Extensionsverband und schließlich die ersten operativen Verfahren, nachdem die Notwendigkeit der Asepsis erkannt und diese praktiziert wurde sowie–schließlich - die röntgenologische Darstellung der Frakturen zur Verfügung stand.
