- Abstract
Objetivo: determinar la frecuencia y grado de invasión de los adenomas hipofisarios funcionales y no funcionales en los pacientes del Instituto Nacional de Neurología y Neurocirugía. Antecedentes: los adenomas hipofisarios (AH) son lesiones del sistema nervioso central (SNC), regularmente benignas. Es una patología relativamente frecuente, con un índice global estimado en la población general de un 16,7% y representa del 10-15% de las neoplasias intracraneales (Araujo et al, 2020); aproximadamente un 25-55% son de tipo invasor (Di Leva et. Al, 2014). Su frecuencia varía ampliamente dependiendo de la edad y el sexo, siendo ligeramente más frecuentes en mujeres con edad de entre los 40 y 60 años (Araujo et al, 2020). Existen varios criterios para determinar si un AH es o no de tipo invasor, ya sea por hallazgos: 1) radiológicos, 2) quirúrgicos, y/o 3) de anatomía patológica (Campero A., Ajler P. 2015). Se ha observado que el patrón de crecimiento de los AH se asocia al tipo de tumor, ya que el crecimiento supraselar es más frecuente en los AH no funcionales (Campero A., Ajler P. 2015) Métodos: se realizó un estudio observacional, retrospectivo, analítico, que incluyo datos recopilados desde 2017 a 2021 de 225 pacientes con diagnóstico de AH del Instituto Nacional de Neurología y Neurocirugía. En cortes de muestras de tumores incluidos en parafina, se realizó tinción con la técnica de inmunohistoquímica contra las hormonas adenohipofisarias para clasificarlos de acuerdo al subtipo de estirpe celular a la que pertenecían los tumores. Para el análisis estadístico se utilizó el programa Graph Pad Prism 8.3.1.471, versión para Windows y se realizó una prueba de t Student no pareada con prueba de Fisher. Núm. de registro del protocolo: 70-18 Resultados: la mayoría de los AH resultaron ser de tipo no funcional (NF) con 179 casos; 186 casos fueron de tipo invasor de estos, 159 de los tumores fueron de tipo invasor NF. Independientemente del sexo la mayoría de los AH fueron de tipo invasor (Figura 1). Se observó una diferencia significativa (p <0.0001) entre el grupo de mujeres con AH NF invasor que es mayor con respecto al grupo de mujeres con AH funcional invasor. El rango de edad de mayor incidencia de los AH tanto para hombre como mujer fue de los 40 a 59 años. El análisis estadístico con la prueba de t no pareada con prueba de Fisher reveló que los AH funcionales (F) de HC (Hormona de crecimiento) fueron significativamente más frecuentes que los NF de HC (p <0.0001). Respecto a la clasificación de Hardy Vezina (HV) los AH más frecuentes se les otorgó el estadio tipo III al momento del diagnóstico con 99 casos, de ellos 86 casos resultaron NF (Tabla 1), 75 casos presentaron una extensión supraselar (Tabla 2) y 65 casos fueron del subtipo C (Tabla 3). Por otra parte, de los pacientes clasificados por KNOSP los de G3 fueron los más comunes (Tabla 4) al momento del diagnóstico. Finalmente en el aspecto clínico se encontró que los síntomas más comunes fueron la cefalea, la disminución de la agudeza visual y la hemianopsia temporal. Las complicaciones pos quirúrgicas más frecuentes fueron la diabetes insípida y la fístula de líquido cefalorraquídeo. Conclusiones: en los casos de adenomas hipofisarios revisados en los pacientes del INNN se determinó que los adenomas hipofisarios más frecuentes son de tipo invasores no funcionales y que en mujeres la presencia del AH no funcional invasor es significativamente mayor que el AH funcional invasor. La frecuencia de edad de la presentación de estos tumores fue en su mayoría de los 40 a 59 años de edad independientemente del sexo. Llama la atención que los pacientes con AH funcionales de HC son significativamente más frecuentes que los no funcionales de este mismo tipo. Por último, cabe destacar que el patrón de crecimiento de los AH estudiados de acuerdo HV, en su mayoría se encuentra orientado hacia la región supraselar y estos a su vez a la porción anterior del tercer ventrículo. Por otra parte de acuerdo con la clasificación de Knosp la invasión se encuentra pasando el límite lateral de la ACI (G3).
