Eficacia de la terapia cognitivo conductual para tratamiento de la depresión en pacientes con epilepsia del lóbulo temporal.

- Abstract
Se calcula que en México hay alrededor de 2 millones de pacientes con epilepsia y que más del 42% sufre además de depresión. El efecto de la depresión sobre la calidad de vida. Puede ser más devastador que la misma epilepsia. Hasta ahora, el tratamiento de la depresión en la epilepsia se ha enfocado al manejo farmacológico, con riesgos por interacciones farmacodinámicas y farmacocinéticas con los antiepilépticos. Objetivo: evaluar la eficacia de la psicoterapia cognitivo conductual (TCC) como tratamiento de la depresión en pacientes con epilepsia del lóbulo temporal. Material y métodos: ensayo clínico con muestreo no probabilístico de sujetos con epilepsia del lóbulo temporal y diagnóstico de trastorno depresivo mayor, según criterios del DSM IV, de la Consulta Externa del Instituto Nacional de Neurología y Neurocirugía de la ciudad de México. 20 pacientes mayores de 18 años ingresaron al estudio, recibiendo 16 sesiones estructuradas semanales de psicoterapia cognitivo conductual de grupo. El diagnóstico psiquiátrico se realizó a través de la entrevista estructurada MINI. La severidad de la depresión y CV se evaluaron a través del inventario de depresión de Beck y la escala de calidad de vida en epilepsia, QOLIE-31. Los instrumentos fueron aplicados antes de iniciar el tratamiento, a la semana 8 y en la última sesión (semana 16). El análisis estadístico, se realizó con el paquete SPSS-13. Resultados: 13 pacientes, con edad promedio de 33 años terminaron las 16 sesiones de TCC, 12 eran mujeres. La edad de inicio de la epilepsia fue de 20 años en promedio. El porcentaje de abandono fue de 35%. El tiempo total de horas/terapia otorgadas fue de 24 hs. A través del análisis lineal para muestras repetidas, se encontró que la mejoría en la severidad de los síntomas de depresión fue estadística significativo (p< 0.00) a partir de la segunda evaluación (semana 8). Si bien se encontró una mejoría en la CV individual en todos los pacientes, esta no fue estadística significativa (p = 0.16). Se realizó también un análisis para determinar qué síntomas de depresión mejoran a través de la TCC, y el efecto sobre cada subescala de CV. Conclusión: la TCC fue efectiva para tratar los síntomas depresivos en pacientes con ELT. También hubo mejoría en la CV, pero no fue estadísticamente significativo, probablemente debido a la principal limitación del estudio, el tamaño de la muestra. De ser confirmados los resultados obtenidos, se deberá considerar la TCC de grupo como una opción más para el tratamiento de la depresión en pacientes con ELT.
