Tkankowa ekspresja receptorów somatostatynowych w raku jelita grubego.

- Abstract
Rak jelita grubego (ang. colorectal cancer, CRC) jest globalnie jednym z najczęstszych nowotworów złośliwych u ludzi. Pod względem zachorowalności zajmuje trzecie, a pod względem śmiertelności -- drugie miejsce. Większość CRC (75-80%) stanowią zachorowania sporadyczne, kształtowane przez czynniki środowiskowe i genetyczne. Zróżnicowaną ekspresję receptorów somatostatynowych (SST1-5) (mRNA, białko) opisuje się głównie w neuroendokrynnych guzach/ nowotworach żołądkowo-jelitowo-trzustkowych. Kwestią otwartą pozostaje natomiast diagnostyczno-prognostyczna rola ekspresji tkankowej SST1-5 w sporadycznym CRC. Zanalizowano immunohistochemiczną (IHC) ekspresję trzech receptorów somatostatynowych (SST2, SST3 i SST5) oraz antygenu Ki-67 w skrawkach parafinowych z materiału operacyjnego (n=30) oraz w dostępnych komercyjnie TMAs (tissue microarrays) chorych na CRC (n=30). Ekspresję tę odniesiono do analizy ekspresji SSTRs w przerzutach CRC do węzłów chłonnych (metastases, meta) (n=30) oraz prawidłowym jelicie grubym (kontrola) (n=58). Do oceny ilościowej nasilenia reakcji IHC na SSTRs wykorzystano metodę morfometryczną (program Filtr HSV). Ekspresję Ki-67 oceniono w 4-pkt skali półilościowej. Wskaźnik wykrywalności SST2/3/5 w CRC wynosił odpowiednio: 100%/88%/100%. Wykazano znamiennie wyższą ekspresję SSTR2 w CRC i w meta w porównaniu z kontrolą (p=0,001, p=0,03, odpowiednio). W każdej z badanych grup wykazano istotnie większą ekspresję SST2 niż SST3 oraz przewagę ekspresji SST5 nad SST3. W CRC pozytywne korelacje dotyczyły wzajemnej ekspresji wszystkich trzech receptorów, a najsilniejsze pomiędzy SST2 z SST5 w CRC, oraz w meta (r=0,790, r=0,780, odpowiednio). W CRC wykazano negatywną korelację ekspresji SST3 z Ki-67 (r=-0,480). Wraz ze wzrostem zaawansowania klinicznego (stage) CRC, rosła ekspresja SST2. Nadekspresja SST2 może stanowić marker diagnostyczno- -prognostyczny CRC. Potencjalną rolę w regulacji proliferacji komórkowej CRC wydaje się odgrywać SST3.
