The sudden death during exercise is an event related almost exclusively to cardiac causes. One of the first case reports of sudden death linked to sport dates back to Greek civilization. According to historical accounts Pheidippides would have been a Greek messenger responsible for running 140 miles from Athens to Sparta to request assistance in order to prevent an imminent Persian invasion. After the historic battle the messenger would have been assigned to run a distance of about 25 miles in order to inform the victory of the Athenian army to his people. Arriving in Athens would have been able to convey the information and, immediately after, come to death. The objective of this study is to address the main causes of sudden death from cardiac etiology for the year, with emphasis on etiology and risk factors in order to rank the most possible causes of death of the Greek messenger, taking into account the aspects of style life and power that would be submitted. searches were conducted without time limits, the virtual databases Public Medline (PubMed), Scientific Electronic Library Online (SciELO) and Virtual Health Library (VHL) and selected case reports, epidemiological bulletins, experimental work and relevant literature reviews. Based on the critical analysis of the data it was concluded that deal between the main base pathologies coronary artery disease, arrhythmias, valvulopathy, Marfan syndrome, congenital abnormalities in the coronary arteries and variants of hypertrophic cardiomyopathy, the latter two who were listed as major probable causes of historical inevitability.
