Atualizações conceituais e medicamentosas nas Diretrizes Brasileiras de Hipertensão Arterial.

- Abstract
Objetivo: Discorrer sobre as principais mudanças presentes na 7ª Diretriz Brasileira de Hipertensão Arterial da Sociedade Brasileira de Cardiologia publicada em 2016, em comparação a anterior, publicada no ano de 2010. Metodologia e fonte de dados: Realizar uma análise comparativa entre as diferenças encontradas na VI e na VII Diretrizes Brasileiras de Hipertensão Arterial disponibilizadas na seção de Publicações científicas do portal da Sociedade Brasileira de Cardiologia no que tange aspectos conceituais e medicamentosos. Discussão: O recomendado pela Diretriz de 2016 é a mensuração a cada dois anos, para os adultos com PA ≤ 120/80 mmHg, e anualmente, para aqueles com PA > 120/80 mmHg e < 140/90 mmHg. As médias de PA de 24h alteraram para ≥ 130/80 mmHg, vigília ≥ 135/85 mmHg e sono ≥ 120/70 mmHg ≤ 120/80 mmHg. História de DCV prematura em parentes de 1º grau e obesidade (IMC ≥ 30 kg/m²) foram adicionados aos fatores de risco cardiovascular. Em idosos com idade ≥80 anos, o tratamento medicamentoso passa a ser indicado a partir de PAS > 160, antes disso, são recomendadas medidas não farmacológicas, como a prática de atividades físicas. Em pacientes com DAC deve-se manter a PA < 130/80mmHg e > 120/70mmHg. Os IECA antecipam os BCC na nova lista das classes de anti-hipertensivos consideradas preferenciais para o controle da pressão arterial em monoterapia inicial. Os BB foram retirados, sendo considerados como fármacos iniciais apenas em situações específicas, devendo estar associados a outros fármacos. Devido ao risco de complicações, a associação de BRA com IECA deve ser evitada em pacientes com HA e DM. Quanto à hipertensão secundária, a chance de desenvolvimento de HA em pacientes com doença renal crônica aumentou para 90%. Foram acrescentadas recomendações gerais para a redução da PA na emergência hipertensiva, são elas: ≤ 25% na primeira hora, PA ≤ 160/100 -110 mmHg em 2-6 h e ≤ 135/85 mmHg em 24-48 h. Conclusão: Devido a alta incidência de Hipertensão Arterial na população mundial torna-se importante a mensuração fidedigna e adequada deste sinal. Uma vez que esta patologia possui um fator clínico multifatorial, a análise conjunta dos fatores predisponentes deve ser realizada. Para tal, o valor de corte adequado, assim como a medicação de referência devem ser estudados e analisados afim de fornecer o melhor prognóstico para o paciente.
