Versorgung primärer Mammakarzinome bei älteren Frauen mit adjuvanter Therapie.

- Abstract
Hintergrund: Brustkrebs ist die häufigste Tumorerkrankung bei Frauen. Der Rückgang der brustkrebsspezifischen Mortalitätsrate in den letzten 15 Jahren ist bei den über 69-Jährigen im Vergleich zu jüngeren Frauen geringer. Altersgruppenspezifische Therapieunterschiede bei der Krebsbehandlung könnten die Ursache dafür sein. Methoden: In einer retrospektiven Analyse wurden Patientinnen, die aufgrund eines invasiven Primärtumors ohne Fernmetastasen eine adjuvante Therapie erhielten, aus den Diagnosejahren 2006-2011 der Patients' Tumor Bank of Hope (PATH)-Datenbank berücksichtigt. Klinische und tumorbiologische Befunde sowie Therapiedaten -- letztere aus dem 2-Jahres-Follow-up -- wurden in drei Altersgruppen (< 50, 50-69, > 69 Jahre) gegliedert. Mittels Chi-Quadrat-Test wurden signifikante Unterschiede geprüft und Post-hoc-Mehrfachvergleiche ohne sowie mit Bonferroni-Korrektur durchgeführt. Therapiedaten wurden für Staging sowie Grading adjustiert und mit einer logistischen Regression auf Altersgruppenabhängigkeit getestet. Ergebnisse: Follow-up-Daten lagen von 3 257 (65 %) der 4 981 Frauen vor, von denen 61 % der mittleren und 22 % der höchsten Altersgruppe angehörten. Patientinnen > 69 Jahre erhielten im Vergleich zu 50- bis 69-Jährigen seltener eine brusterhaltende Therapie (68,8 versus 86,4 %; p < 0,001), eine Chemosowie Strahlentherapie (27,5 versus 44,1 % und 81,8 versus 92,4 %; p < 0,001) und Trastuzumab (52,9 versus 79,3 %; p < 0,001). Nach Adjustierung blieben diese Unterschiede signifikant. Schlussfolgerungen: Die Ursache für die beobachteten Altersgruppenunterschiede ist unklar. Die Frage, ob Komorbiditäten oder andere Faktoren wie beispielsweise die individuelle Entscheidung der Patientin für den Therapieverzicht verantwortlich sind, kann nicht beantwortet werden.
