Allogene Gesichtstransplantation: Fiktion oder Realität?

- Abstract
Zusammenfassung Hintergrund Ziel der Arbeit ist die Beschreibung möglicher Formen einer allogenen Gesichtstransplantation und die Darstellung der Kompatibilität des Spenders und des Empfängers am Leichenmodell. Methoden Am Leichenmodell wurden basierend auf der Blutversorgung der externen Gefäße eine reine Hautlappenplastik (Typ 1) und eine kombinierte osteokutane Lappenplastik (einschließlich Le-Fort-III-Knochensegment, Typ 2) entwickelt. Ergebnisse Beide Lappenplastiken beruhen auf der externen Blutversorgung der Fazialisgefäße, der Temporalisgefäße und der V. jugularis externa. Die sensible Versorgung der Typ-1-Lappenplastik erfolgt theoretisch durch die Endäste des N. trigeminus, bei der Typ-2-Lappenplastik ist die Wiederherstellung der Mimik durch den Einschluss des N. facialis möglich. Die Spender-Empfänger-Kompatibilität wird durch Hautfarbe und Textur, anthropometrische Schädeldimension, spezielle Weichteilkomponenten (Nase, Lippe, Wange, Augenbrauen) und Geschlecht determiniert. Schlussfolgerung Neben wesentlichen ethischen Fragen bleibt die erforderliche langfristige Immunsuppression der wichtigste potenziell limitierende Faktor einer zukünftigen kompletten allogenen Gesichtstransplantation am Menschen.
