- Abstract
Hintergrund: Die mechanische Beatmung erfolgt in der Intensivmedizin häufig und ist oft eine lebensrettende Intervention. Circa 35 % der Intensivpatienten werden mechanisch beatmet, allerdings entwickeln etwa 15 % dieser Patienten eine beatmungsassoziierte Lungenentzündung. Ziel der Beatmungstherapie ist es, die Atemarbeit zu senken und den pulmonalen Gasaustausch und damit die Versorgung des Organismus mit Sauerstoff aufrechtzuerhalten oder wiederherzustellen. Es sind zahlreiche Beatmungsmodi verfügbar, wobei in den letzten Jahren die Vermeidung der beatmungsinduzierten Lungenschädigung durch lungenprotektive Beatmungsstrategien in den Mittelpunkt des Interesses gerückt ist. Methode: Es wurde eine selektive Literaturrecherche durchgeführt. Ergebnisse: Im Vergleich zu konventionellen lungenprotektiven Beatmungsformen stellen die in dieser Übersichtsarbeit vorgestellten modernen Beatmungsformen Weiterentwicklungen dar, die die Lungenprotektion optimieren und gleichzeitig die Lungenfunktion sowie die Synchronie zwischen Patienten und Beatmungsgeräten verbessern. Bei der Hochfrequenzbeatmung werden Tidalvolumina von 1-2 mL/kg (Körpergewicht) bei einer Atemfrequenz von bis 12 Hz eingesetzt. Darüber hinaus werden assistierte Formen der Spontanatmung wie „proportional assist ventilation“, „neurally adjusted ventilatory assist“ oder variable Druckunterstützungsbeatmung eingesetzt. Computergesteuerte „closed-loop“-Beatmungssysteme ermöglichen eine automatisierte Beatmung und können die Zeit bis zur Entwöhnung vom Beatmungsgerät gemäß einer aktuellen Metaanalyse um 32 % verkürzen. Schlussfolgerung: Die wissenschaftliche Evidenz hinsichtlich klinisch relevanter Endpunkte ist für alle modernen Beatmungsformen noch unzureichend. Neue Beatmungsformen scheinen jedoch die Beatmungsinvasivität beziehungsweise die Beatmungsdauer zu reduzieren und könnten somit die Therapie beatmungspflichtiger Patienten verbessern. Um eine abschließende Beurteilung moderner Beatmungskonzepte zu ermöglichen, sind randomisierte Studien mit klinisch relevanten Endpunkten notwendig.
