Utilização de firocoxibe no pós-operatório de celiotomia em neonato com compactação por mecônio.

- Abstract
Uma das causas mais comum de síndrome cólica em neonatos é a retenção e compactação do mecônio. Constituído de líquido amniótico digerido, debris celulares e secreções glandulares do trato gastrointestinal, o mecônio, fisiologicamente, é eliminado entre 2-12 horas após o nascimento. O presente estudo relata o desfecho cirúrgico para a resolução de compactação devido à retenção por mecônio em um potro neonato com 24h de vida, com ênfase na utilização de firocoxibe na terapia pós-operatória. O paciente da raça Puro Sangue Inglês, encaminhado ao HCV-UFPel, não teve um parto assistido. A égua foi encontrada solta pela manhã com sinais de pós-parto, mas sem estar junto ao potro. Algumas horas depois, o potro foi encontrado. No mesmo momento, a amamentação foi estimulada, mas ainda assim o potro não ingeriu o colostro nas primeiras horas de vida e não observou-se eliminação de mecônio. Cerca de 6 horas depois, ainda na propriedade, o paciente apresentou sinais de desconforto abdominal. Um atendimento prévio foi realizado com fluidoterapia, enema com solução fisiológica aquecida e analgesia utilizando dipirona (20 mg/kg). Não tendo uma evolução clínica favorável do quadro, o potro foi referenciado ao HCV. No exame clínico inicial, o animal apresentava frequência cardíaca de 120 bpm, frequência respiratória de 60 mpm, desidratação de 5%, temperatura de 38,3 °C, motilidade reduzida em todos os quadrantes, bem como sinais de dor. Ao realizar-se a avaliação ultrassonográfica (US), para avaliação foi realizado o protocolo FLASH, no qual observou-se no abdómen ventral intestino delgado com alças distendidas e sem motilidade, e na janela gástrica estômago repleto (Janela 2). Não foram observadas alterações nas demais porções avaliadas. Em virtude da suspeita de obstrução intestinal, o paciente foi encaminhado ao bloco cirúrgico para uma celiotomia exploratória. Induziu-se a anestesia utilizando cetamina e diazepam; para a manutenção anestésica, utilizou-se isofluorano. Realizou-se uma incisão na linha média do abdomem; após exploração, foi encontrada uma porção de cólon menor compactado. Após a retirada da massa, realizou-se a síntese do cólon reposicionado na cavidade e foi feita a síntese do abdomem. No pós-operatório, optouse pelo uso de firocoxibe (0,1 mg/kg, q24h, IV, 6 dias) como único agente anti-inflamatório. O firocoxib, que é um AINE pertencente ao grupo dos coxibes, atua por inibição seletiva da síntese da prostaglandina mediada pela cicloxigenase-2 (COX-2). Por ser uma medicação seletiva para COX-2, apresenta menos efeitos adversos uma vez que COX-2 é a isoforma induzida apenas em sítios de inflamação e em poucos tecidos. Desta forma, tem ação apenas nos sítios inflamatórios, sendo que a farmacocinética e a segurança da administração oral de firocoxib já foram testadas, mostrando maior segurança quanto aos outros AINEs no seu uso em potros. Apesar de ser utilizado como rotina no exterior, no Brasil ainda é pouco explorado. Após seis dias de internação e com a recuperação clínica completa, o animal recebeu alta. A utilização de apenas o firocoxibe no pós-operatório se mostrou eficiente no controle da dor e processo inflamatório, assim como segura devido à ausência de efeitos colaterais.
