Leprosy is an ancient infectous contagious disease caused by the Mycobacterium leprae, which has a predilection to affect skin and preripheral nerves. The disease has cure and treatment available for free in Brazil through the Unified Health System (SUS). Despite that, in Brazilian northeast region, Maranhão continues to be the second most affected state in Brazil. OBJECTIVES: Study the epidemiological profile of leprosy on the period of 2016 to 2020 and determine the most relevants epidemiological indicators. METHODS: Collect of data was made through the Derpartment of Informatic of the Unified Health System (DATASUS). Determination of indicators relacionated to the quality of services and elimination indicators were realized in accordance to the Tecnical-Operational Manual from Health Ministry. RESULTS: Were related 14920 new cases, 1422 under 15 years old and the groups most affected were: Men (56,97%), 30-59 years (48,81%), browns (67,73%) and incomplete elementar school (634 cases/years). The majority had grade 0 of disability (54,92%), the Multibacillary form (79,02%) and Dimorphic clinical apresentation (56,22%). The most affected cities were São Luís, São José de Ribamar and Imperatriz. Maranhão was classified as a hyper endemic region (2016-2019) and with a very high prevalence (2020). From the 4 indicators evaluteds related to the quality of service, one was qualified as good, two regular and one precarious (2016-2020). CONCLUSIONS: Maranhão persists as a Brazilian region with a high prevalence of leprosy, however, an improvement in the quality of services offered could contribute to the reduction of this epidemiological picture.
