The article focuses on the role of positive end expiratory pressure (PEEP) in patients with acute respiratory distress syndrome (ARDS). Topics discussed include the limitations of esophageal pressure for estimating pleural pressure in critically ill patients, and issues when setting PEEP from directly measured esophageal pressure.
