- Abstract
Introduction: Over malignancies attributed to the 2% of women deaths are usually presented as germ cell tumors in young women. Because of the chance in undergoing fertility preserving surgery, the early diagnosis of over malignancies is important. So, we aimed to investigate the histopathological manifestations of over malignancies incidentally found out during cesarean sections (C/S). Material And Methods: The data of totally 11.154 patients undergone C/S at Etlik Zübeyde Hanım Education and Research Hospital between January, 2007 and March 2009 were investigated retrospectively. The demographic data, mass size, location and the histopathological feature of mass, and the complications happened due to adnexial mass during the pregnancy and delivery of 78 patients presented with adnexial mass intra-operatively were collected. Results: The mean size of masses detected in 78 patient was 4 cm (1-15 cm). The most common localization was para-ovarian section. Bilateral adnexial mass was seen in only three patients (3.9%). Five patients undergone cyst aspiration, 50 patients undergone mass excision, and three patients had unilateral salphingo-ooferectomy due to the mass size and localization. All of the cytological examinations revealed benign results, but two patients undergone cyst excision and undergone following fertility preserving surgery due to the presence of borderline epitelial ovarian cancer found out by the histopathological procedures. The most common adnexial pathological finding was serous cyst adenomas (21.5%). The incidence of malignancy was found to be as 0.18 per 1000 C/S. The complication rate of adnexial masses was 6.4%. Conclusion: Due to small number of patient, our results did not support the literature recommend physicians not to forget checking out adnexial locations during C/S. On the other hand, it is believed that suspicious masses should undergo frozen sectioning leading to early diagnosis of ovarian malignancies which give a chance to the patients undergoing fertility preserving surgery and to avoid second procedures taken after delivery. We need prospectively designed further studies to focus this problem.
