Reconstrução cirúrgica do osso frontal e teto de órbita: relato de caso.

- Abstract
Introdução: O osso frontal localiza-se em uma região nobre e compõe-se de estruturas importantes, como seio frontal e ducto nasofrontal, além de formar o teto da órbita e envolver a parte anterior do cérebro. Fraturas do osso frontal tem maior incidência no sexo masculino e pacientes jovens, estando associadas a acidentes de alto impacto, como acidentes automobilísticos, agressão física e ferimento por arma de fogo. Objetivo: Apresentar um relato de caso de agressão física que resultou em fraturas de teto de órbita e seio frontal. Conduta: Paciente do sexo masculino, 40 anos, deu entrada no pronto socorro do hospital da Santa Casa de Araçatuba, após ser vítima de agressão física. Ao exame físico extraoral evidencia-se um ferimento abrasivo em região superciliar esquerda. A apalpação, verificavase um degrau ósseo em região supraorbitária e frontal esquerdas. A acuidade visual apresentava-se preservada, a mobilidade ocular com movimentos de lateroversão e supraversão se apresentaram limitados. Foi solicitado como exame de imagem, uma tomografia computadorizada de face, confirmando o diagnóstico clínico de fratura de parede anterior de seio frontal com deslocamento e fratura do teto da órbita esquerda. Resultado: O tratamento proposto e realizado foi a realização de um acesso coronal para visualização adequada, redução e fixação dos fragmentos ósseos com placas do sistema 1,5mm, reconstrução do teto e perímetro da órbita com malha de titânio. Conclusão: Em suma, um acesso amplo e o uso de placas e malha de titânio promove o restabelecimento da função e da estética do paciente, proporcionando o sucesso do tratamento cirúrgico, além de ser viável e não apresentar intercorrências.
