MRT bei Patienten mit Hörimplantaten mit Magnetausstattung - ein Update.

- Abstract
Hintergrund: In Deutschland leben mehr als 100.000 Patienten mit einem Cochlea-Implantat und weitere Patienten mit magnetversorgten Knochenleitungs-, Mittelohr- und Hirnstammimplantaten bei zunehmenden Indikationen zur MRT-Untersuchung.Methoden: Dieses Update informiert über die in der Europäischen Union und in den USA seit November 2013 medizintechnisch neu zugelassenen magnetversorgten Hörimplantate und Produkterweiterungen sowie über die verbindlich vorgeschriebenen Sicherheitsvorschriften. Einbezogen wurden alle unter einer selektiven Literaturrecherche in PubMed mit den Suchbegriffen „MRI-compatibility"/„MRI-safety" + „cochlea implant", „auditory brainstem implant", „BAHA Attract"/„Bonebridge", „Vibrant-Soundbridge"/„Sophono alpha" aufgefundenen Artikel.Ergebnisse: Die bei der MRT-Untersuchung notwendigen hohen elektromagnetischen Feldstärken können selten zu Komplikationen führen. Bildartefakte in der MRT-Schädel-Untersuchung können durch die Modifizierung der Aufnahmetechnik vermindert werden. Alle neu zugelassenen magnetversorgten Hörimplantate sind für die MRT-Untersuchung mit Feldstärken bis inklusive 1,5 T zugelassen, einige Implantate bis zu 3,0 T Feldstärke. Die im Artikel von Nospes et al. vom November 2018 aufgeführten notwendigen Sicherheitsmaßnahmen für ältere Hörimplantate sind unverändert zu beachten.Schlussfolgerungen: MRT-Untersuchungen sind mit den dafür zugelassenen magnetversorgten Hörimplantaten bei Belassen des implantierten Magneten ohne Gefährdung der Patienten möglich. Eine MRT-Untersuchung sollte aber bei diesen Patienten weiterhin nur bei strengster Indikationsstellung und in enger Absprache mit dem den Patienten betreuenden otologischen Team erfolgen. Zuvor sind alle anderen alternativen diagnostischen Möglichkeiten auszuschöpfen.
