- Abstract
Introducción. La enfermedad por coronavirus 2019 (COVID-19) es causada por el betacoronavirus SARS-CoV-2, un virus con genoma de ARN de cadena sencilla de polaridad positiva y extensión de aproximadamente 30.000 bases. Desde el principio de la pandemia la caracterización del componente genético de las variantes circulantes ha sido importante para conocer aspectos como la capacidad de dispersión viral, el mejoramiento del diagnóstico, el diseño de vacunas y para comprender desde una aproximación viral la dinámica de la COVID-19, más aun considerando la aparición de variantes de preocupación y de interés del SARS-COV-2. Objetivo. Implementar la vigilancia genómica rutinaria del SARS-CoV-2 para orientar la toma de decisiones en salud pública en Colombia. Materiales y métodos. Se hizo una encuesta de capacidades técnico-científicas de universidades, centros de investigación y empresas con experiencia en genómica. Se establecieron convenios de cooperación, se entregaron reactivos y se entrenó al personal en secuenciación del genoma completo del SARS-CoV-2 mediante la tecnología Oxford Nanopore. El genoma completo se secuenció utilizando secuenciación de amplicones según el protocolo Artic Network. Para la asignación de linajes se utilizó el algoritmo PANGOLIN. Resultados. La vigilancia genómica permitió reconstruir la historia del SARS-CoV-2 en el país. Se encontró que por lo menos 12 linajes diferentes del virus se introdujeron al inicio de la pandemia en Colombia. Asimismo, fue posible determinar la distribución porcentual de los linajes en diferentes momentos de la pandemia, así como el reemplazo de variantes. Con la información genómica se evaluó la relación entre la variabilidad genética y la gravedad de la enfermedad para contribuir al esclarecimiento de algunas de sus dinámicas particulares. Se hizo seguimiento a las sustituciones en genes de importancia como el que codifica la proteína Spike y se estudió la variabilidad genética local, lo que permitió el refinamiento de oligonucleótidos de detección molecular para el diagnóstico de la COVID-19 y el diseño de péptidos para proponer una prueba Elisa in house. Conclusiones. La vigilancia genómica del SARS-CoV-2 en Colombia ha posicionado al país como un referente latinoamericano en estudios genómicos en tiempo real y en diferentes enfoques de investigación básica y aplicada para orientar las decisiones en salud pública.
