- Abstract
Giriş: Bu çalışmada kan kültürlerinde Anginosus grup streptokok (AGS) üreyen olguların klinik özelliklerinin değerlendirilmesi ve eşlik eden komorbiditelere dikkat çekilmesi amaçlanmıştır. Gereç ve Yöntem: Çalışmada; 2009-2012 yılları arasında İzmir’de 3. basamak bir hastanenede yatan hastalardan gelen kan kültürlerinde Bactec 9240 kullanılarak AGS saptanan 8 izolat ve hastaların özellikleri değerlendirmeye alındı. Hastaların demografik verileri, klinik bulguları ve eşlik eden komorbiditeler analiz edildi. Bulgular: Kan kültürlerinde AGS üreyen ve enfeksiyon hastalıkları servisinde izlenen iki hasta sırasıyla enfektif endokardit ve akut pürülan meningoensefalit tanıları aldı. Hemodiyaliz ünitesinde izlenen bir hastada santral venöz kateter enfeksiyonu saptandı. Kan dolaşımı enfeksiyonu tanısı alan iki hastadan biri transplantasyon cerrahisi bölümünde izlenen bir böbrek alıcısı, diğeri ise dahiliye servisinde izlenen bir KML hastasıydı. Kan kültürlerinde AGS üreyen üç hastada sırasıyla beyinde iskemik atak sonrası gelişen enfekte hematom, travmaya bağlı multiple kafa kaide kırıkları ve anorektal apse drenajı sonrası gelişen cerrahi sepsis tanıları mevcuttu. Akut pürülan menenjit ve kafa travması tanılı hastalarda AGS’lere eşlik eden anaerobik etkenler de saptandı. Sonuç: AGS’ler fırsatçı patojen bakteriler olup literatürde genellikle invaziv apseiform lezyonlarla seyrettiği bildirilmektedir. İntraabdominal ve intrakraniyal süpüratif odaklar primer enfeksiyon kaynakları olabilir. Ayrıca hematolojik maligniteler, solid organ tümörleri, geçirilmiş cerrahi girişimler, ve immünosüpresyon bu tür enfeksiyonlara yatkınlık oluşturmaktadır. Kan kültürlerinde AGS üremesi durumlarında klinisyenin bu durumları göz önünde bulundurması önemlidir.
