EVALUACIÓN DE LA EXPRESIÓN DE LOS FACTORES DE TRANSCRIPCIÓN EN LOS TUMORES NEUROENDÓCRINOS DE LA ADENOHIPÓFISIS Y SU RELACIÓN CON LOS MARCADORES DE PROLIFERACIÓN.

- Abstract
Objetivo: Analizar la expresión de los factores de transcripción (FT) en los tumores neuroendócrinos de la adenohipófisis y su relación con la proliferación celular, como un posible marcador de su comportamiento. Antecedentes: Los adenomas hipofisarios (ADH) son considerados benignos, sin embargo pueden ser invasores y agresivos. Los FT participan en la diferenciación de los tipos celulares hipofisarios y su sobreexpresión en ADH sugiere que juegan un papel importante en la formación de estos tumores. Métodos: En cortes de 4 µm de ADH (proyecto 24/06) se detectó por inmunohistoquímica la expresión de las hormonas adenohipofisarias (Prl, HC, FSH, LH, TSH y ACTH), Ki67, p53, PCNA, Ciclina D1 (CD1), PTTG, Pit1, ERa, SF1. Se cuantificó el índice (I) de marca (% de células positivas en 10 campos) y se realizó análisis estadístico (ANOVA seguida por Kruskal-Wallis y U Mann Whitney y análisis de correlación con la prueba de Spearman. Resultados: Se analizaron 85 casos (edad 48.6±12.85), 48 mujeres (56.5%); 69 (81.2%) clínicamente no funcionantes (CNF) y 16 (18.8%) funcionantes (CF); 32 positivos para Prl y/o HC (16 CF y 16 CNF) y 53 para FSH y/o LH, ambos grupos con positividad sola o en combinación con otras hormonas; 7 casos (8.3%) no secretores. El I de p53, CD1, ERa y Pit1 fue mayor en los CF (Tabla 1), con correlación entre Ki67 con PTTG (P= 0.002 r2=0.665) y entre p53 con PTTG (p= 0.006 r2= 0.615). En CNF correlacionó: ERa con Ki67 (p= 0.011, r2=0.276), p53 (p= 0.037, r2= 0.216) y PCNA (p=0.016, r2= 0.258); SF1 con p53 (p= 0.001, r2= 0.378), PCNA (p= 0.0003, r2= 0.427) y CD1 (p=0.000, r2= 0.506); Pit1 con p53 (p= 0.016, r2= 0.258); PTTG con PCNA (p= 0.025, r2=0.238). Los Prl-HC mostraron diferencia entre CF y CNF en ERa y Pit1; en los CF correlacionó PTTG con Ki67 (p= 0.002, r2=0.665) y con p53 (p= 0.006, r2= 0.615); y en los CNF correlacionó ERa con Ki67 (p= 0.005, r2= -0.415) y con p53 (p= 0.009, r2= 0.579). En FSH-LH ERa correlacionó con p53 (p= 0.003, r2= 0.651); SF1 con p53 (p=0.009, r2= 0.666), PCNA (p=0.009, r2= 0.668) y CD1 (p= 0.001, r2= 0.822); PTTG con Ki67 (p= 0.047, r2= 0.433). Conclusiones: La expresión de los FT identifica la línea celular de origen de los ADH, permite realizar la sub-clasificación histopatológica. Hasta ahora no es clara la relación entre el tamaño tumoral, el tipo hormonal y su comportamiento. Aquí se muestra que puede existir correlación entre el tipo tumoral, los FT y la actividad proliferativa de estos tumores.
