99: The effects of intravesical chemoimmunotherapy with gemcitabine and bacillus Calmette-Guérin for prophylaxis of recurrence of superficial bladder cancer.

- Abstract
Introduction: We prospectively studied the effects and the safety of intravesical chemoimmunotherapy with gemcitabine and Bacillus Calmette-Guerin (BCG) for prophylaxis of recurrence of superficial bladder cancer (pTa, pT1) and compared with those of BCG monotherapy. Methods: Between May 2005 and April 2007, 87 superficial bladder cancer patients were enrolled this study and treated by chemoimmunotherapy (37) or conventional 6 weeks BCG monotherapy(50). For the chemoimmunotherapy, gemcitabine(1000 mg) was instilled immediately after transurethral resection(TUR) and one week later (2000 mg). At 2 week after TUR, Tice-strain BCG was instilled into the bladder once a week for 6 weeks. Side effects, recurrence rates, progression rates and recurrence-free period (RFP) were analyzed in both groups. Results: The recurrence rate was 24.3%(9/37) in patients with chemoimmunotherapy and 30.0%(15/50) in BCG monotherapy patients (P=0.368). But the recurrence-free period (RFP) of chemoimmunotherapy group (11.7 months) was significantly longer than that of BCG monotherapy group (6.7 months, P=0.013). Progression rates of two groups were 8.1% and 10.0%, respectively (P=0.535). Treatment was well tolerated and most of complications were self-limiting in both group patients. Conclusions: Intravesical chemoimmunotherapy with gemcitabine and BCG seemed to be effective for prolongation of the recurrencefree period than BCG monotherapy in superficial bladder cancer patients but cancer recurrence rate was not significantly different between two groups.
