- Abstract
Introducción La hospitalización es un momento de extrema vulnerabilidad para los adultos mayores frágiles. Hay poca evidencia sobre la eficacia de la cogestión geriátrica o de intervenciones de cuidados transicionales en América Latina. Nuestro objetivo fue evaluar si la cogestión geriátrica combinada con una intervención interdisciplinaria podría reducir la tasa de reingreso hospitalario a los 30 días respecto de la atención habitual en pacientes mayores frágiles hospitalizados en una institución de tercer nivel de atención en Argentina. Métodos Diseño: Ensayo clínico controlado aleatorizado simple ciego. En la rama de tratamiento de atención habitual (control) todos los procedimientos realizados durante la hospitalización eran supervisados por un especialista en medicina interna y se ajustaban a protocolos predefinidos. Los pacientes tenían acceso a la atención de un especialista si era necesario, así como a servicios de internación domiciliaria o atención primaria basada en el hogar si correspondía luego del alta hospitalaria. En la rama de tratamiento del programa de gestión clínica en geriatría (intervención), además de la atención habitual, un equipo de co-manejo geriátrico realizaba una evaluación geriátrica integral durante la hospitalización, proporcionaba recomendaciones a medida para minimizar los síndromes geriátricos y planificaba la transición de los cuidados. Un orientador socio-sanitario supervisaba la continuidad de los cuidados en los hogares de los pacientes tras el alta. Resultados Se incluyeron 120 participantes en cada rama. Los reingresos hospitalarios a los treinta días fueron un 47,7% menores en la rama intervención (18,3% frente a 35,0%; P = 0,040); y las visitas a urgencias en los primeros 6 meses después del alta fueron un 27,8% menores (43,3% frente a 60,0%; p = 0,010). Hubo una disminución no estadísticamente significativa de la mortalidad a los 6 meses en el grupo de intervención (25,0% frente a 35,0%; p = 0,124). Discusión Nuestro estudio está en concordancia con otros internacionales en los que las intervenciones que se iniciaron durante la hospitalización y continuaron luego del alta fueron más eficaces para reducir los reingresos que las intervenciones que se iniciaron después del alta. Conclusiones La cogestión geriátrica de personas mayores frágiles durante la hospitalización, combinada con una intervención interdisciplinaria de cuidados transicionales, redujo los reingresos hospitalarios de 30 días y las visitas a urgencias 6 meses después del alta. Lecciones aprendidas La coordinación del geriatra durante la internación y del orientador socio-sanitario luego del alta fueron un componente clave para el adecuado funcionamiento del programa. Limitaciones El estudio simple ciego hace que no se puede descartar el sesgo de desempeño de los clínicos asistenciales. No obstante, éstos no formaban parte del equipo de investigación y desconocían los desenlaces del estudio que se estaba realizando. Además, no encontramos desviaciones de los protocolos habituales de atención y los resultados fueron evaluados por un investigador independiente que no conocía las ramas asignadas. Sugerencias para futuras investigaciones Realizar un estudio multicéntrico con mayor tamaño muestral y más tiempo de seguimiento para evaluar con un poder suficiente la efectividad respecto de mortalidad, tanto a corto como largo plazo.
