- Abstract
Diabetes is an empidemy of XXI century and connected to it morbidity is still rising. There are nearly 400 million people affected by this disease. Estimates say that in Poland it affects more than two million people. Diabetes favors development of atherosclerotic plaques in arteries of the whole body, causing both macro- and microangiopathy. Studies show that diseases of the cardiovascular system, which are developed on the basis of existing atherosclerotic lesions are responsible for nearly half of the deaths in people with diabetes. One of the complications of long lasting diabetes is presence of lower limb ischemia, which can further promote the formation of not only difficult to heal wounds, but also it can be a direct cause of a limb amputation. Among these patients the most important thing is an early detection of the most endangered people. So far existing classifications, such as Wagner's classification, PEDIS, and Texas are not sufficient in a full risk assessment of patients with a chronic wound in diabetes, because they does not take into account the most important factors that may affect the possibility of amputation. In 2014, the Society for Vascular Surgery (SVS) published a new classification - Wound, Ischemia and foot Infection (WIfI), taking into account factors that might directly threaten the survival of the limb. This classification determines whether the extent of the wound, presence of gangrene, the degree of a limb ischemia, or the presence and severity of foot infection. On this basis it is possible to determine the risk of amputation, as well as the necessity and benefits of revascularization procedures, not only of people with ulcers connected to the diabetic foot, but also of people with ischemic rest pain or difficult to heal ulcers of the limb resulting in different etiology.
