- Abstract
OBJECTIVE To investigate if the cause of Meniere disease is peripheral-cochlear or if it has a central component. PATIENTS AND METHODS Retrospective study- analyzing the responses of central afferent auditory system in 36 patients with vertigo and imbalance, fluctuating hypoacusis and tinnitus, who were clinically considered with the classic Meniere disease. They were studied with the usual otoneurological exams and central afferent auditory way was studied with the averaged auditory brain mapping. This study was done in all patients by sonorous stimulation of an ear and then afferent responses were recorded on the brain cortex till 15 mileseconds, by a computerized electroencephalograph of 22 channels; responses were averaged to eliminate nun auditory brain activity, which permitted, by quick Fourier transform, to do a topographic map of the electrical responses of the brain stem and the brain zones processing audition, A n ear was first examined and then the other. Brain map obtained in 36 patients with unilateral Meniere disease, repeated in the different phases of the auditory fluctuation, were compared to those obtained from healthy persons and without auditory diseases. RESULTS In all patients with Meniere disease it was found afferent signal disorder in the contra lateral primary auditory brain cortex and the ipsilateral secondary auditory cortex. CONCLUSIONS This response of the auditory brain cortex, different from that found in persons without this disease, could be useful to find the causes of Meniere disease.
