- Abstract
INTRODUÇÃO Pacientes com em hemodiálise (HD) podem cursar com desnutrição energético-proteica (DEP). O suporte nutricional durante a sessão de HD torna-se importante para evitar a DEP. É comum a oferta de lanches durante a sessão de HD, no entanto costumam ser compostos por produtos ultraprocessados (UPF) que apresentam alto teor de sódio, fósforo e potássio. Dessa forma, torna-se importante desenvolver alternativas de lanches durante a sessão de HD com boa aceitação, nutricionalmente adequados e de custo acessível. O objetivo desse estudo é avaliar se um lanche intradialítico composto por minibolo caseiro e com alto conteúdo energético-proteico é uma alternativa viável para o lanche de HD. MÉTODOS O desfecho primário será o aumento da albumina sérica após a intervenção. Para tanto, um ensaio clínico randomizado e controlado com duração de 24 semanas (primeiras 12 semanas com a intervenção (T12) e as 12 semanas seguintes para acompanhamento sem a intervenção (T24)) foi planejado. Serão incluídos 82 adultos pacientes em HD > 3 meses (COEP HUPE n. 4.568.844 de 2 de março de 2021). Os pacientes serão randomizados em Grupo Proteico (GP) que receberá minibolo proteico contendo proteína do soro do leite e, Grupo Controle (GC) que recebera o minobolo padrão. O lanche será composto por 2 unidades de minibolo de ervas, 10 g de azeite de oliva e 1 copo de 140 mL de água (GP: 506 kcal e 12 g proteína; GC: 500 kcal e 6 g proteína). A albumina sérica, composição corporal, sintomas de desconforto gastrointestinal, bem como da pressão arterial diastólica (PAD) e sistólica (PAS) serão investigados em três momentos: linha de base (T0), após 12 semanas de intervenção (T12) e após 12 semanas de acompanhamento (T24). As comparações entre os grupos e entre os momentos do estudo serão realizadas pelo teste ANOVA de análises repetidas para amostras pareadas. RESULTADOS Serão apresentados os resultados do estudo piloto que incluiu 10 pacientes, 5 em cada grupo. A aceitação do minobolo foi avaliada pelo resto-ingestão na primeira semana do estudo em 3 sessões de HD e não demostrou diferença estatística entre os grupos. A idade (GP: 68 ±22 e GC: 58 ±20 anos), distribuição de sexo masculino (GP: 60% e GC: 40%) e o estado nutricional (avaliação global subjetiva de 7 pontos; GP: 5,6 ±1,5 e GC: 5,8 ±2 pontos) não diferiram significativamente entre os grupos, mas as médias apontaramalgum grau de DEP. Nenhum dos parâmetros avaliados modificaram ao longo do estudo (albumina, potássio, fósforo e ureia séricas, peso corporal, composição corporal, PAD e PAS antes e após a HD da primeira semana no T0, T12 e T24 meses). CONCLUSÃO Os resultados do estudo piloto apontaram que o desenho de estudo está adequado, uma vez que o lanche proposto foi bem aceito, não promoveu aumento de exames laboratoriais importantes para o controle da DRC (como potássio, fósforo e ureia sérica) e não causou alterações arteriais pressóricas durante a sessão de HD ou desconforto gastrointestinal.
