- Abstract
Epidemiology of invasive fungal disease (IFD) provides data on increasing rate of resistant strains with respect to all classes of antifungals. Response to antifungal therapy depends on many factors, including those which can be modified, such as antifungal stewardship (AFS). It is the strategy comparable to the process of antimicrobial stewardship (AMS). In the context of practical general principles, AMS can be expressed by the rule of 6D: diagnosis, debridement/drainage, drug, dose, de-escalation, duration. The concept of rationalizing of clinical activity and avoiding of mistakes in everyday medical practice has already own history in medicine. In 2012 in USA the campaign "Choosing Wisely" was started. Over the last decades the hematology, oncology and transplant settings are still facing the unresolved problem of IFD in spite of significant progress in diagnostics, prophylaxis and therapy. In May 2021, at X Congress of Polish Society of Pediatric Oncology and Hematology, a specific Antifungal Stewardship Program Cure the patient! was announced by the Society. Four main objectives of the Program determine: rationale use of antifungals, decreasing antifungal resistance of fungi, avoiding mistakes in clinical practice, education of medical staff, patients and their families. Concept of Antifungal Stewardship Program is based on six principles determined by acronym WYLECZ! (Cure the patient!), used from Polish words, respectively: diagnosis, debridement, drug, dose, duration, debriefing.
