- Abstract
Aim The objective of this study was the analysis of the efficacy of prophylaxis and therapy of invasive fungal disease (IFD) with micafungin in children undergoing intensive chemotherapy for malignancy (pediatric hemato-oncology - PHO) or hematopoietic stem cell transplantation (HSCT) in Poland between 2012-2015. Material and methods Efficacy of micafungin was analyzed with respect to prophylaxis and therapy of IFD. A total number of 231 patients (181 PHO, 50 HSCT), including 93 patients (63 PHO, 30 HSCT) treated in 2012-2013 and 138 (118 PHO, 20 HSCT) treated in 2014-2015. Results Overall success rate of therapy with micafungin was 82.4% in empirical therapy (in FUO cases), in 80% of preemptive therapy (including 82.5% possible IFD and 70.3% probable IFD) and in 81.5% targeted therapy. In PHO patients success rate of micafungin in empirical therapy was 81.3%, in therapy of possible IFD 86.9% and 86.2% in respective time periods, in therapy of probable IFD - 77.8% and 100%, while in therapy of proven IFD 87.5% and 75%, respectively. In HSCT patients success rate of micafungin in empirical therapy was 82.4%, in therapy of possible IFD 53.3% and 85.7% in respective time periods, in therapy of probable IFD - 50% and 66.7% while in therapy of proven IFD 100% and 50%, respectively. Conclusions Efficacy of micafungin in empirical, preemptive and targeted antifungal therapy in children with malignancy or undergoing hematopoietic stem cell transplantation was estimated to be over 80%. A better efficacy of antifungal therapy was observed in patients undergoing conventional chemotherapy.
