- Abstract
Estudios recientes de población y series de caso sugieren que la enfermedad vascular cerebral (EVC) y los factores de riesgo vasculares contribuyen a la heterogeneidad de las alteraciones cognoscitivas. En los casos de demencia vascular las manifestaciones incluyen pérdida de memoria, confusión mental, y alteraciones ejecutivas, además se presentan alteraciones "focales" circunscritas de una función especializada como afasia, apraxia y agnosia. Objetivos: describir y comparar las ejecuciones cognoscitivas pos EVC en pacientes del Instituto Nacional de Neurología y Neurocirugía. Material y métodos: es un estudio longitudinal donde se evaluaron 110 pacientes, mayores de 18 años con diagnóstico de EVC isquémico, hemorrágico o trombosis venosa cerebral, con un periodo de evolución de tres meses. A todos los pacientes se les realizó estudios de imagen, genéticos y una valoración neurológica, neuropsicológica y neuropsiquiátrica inicial (3 meses) y una de seguimiento (12 meses), con el fin de corroborar el deterioro cognoscitivo. En el seguimiento se evaluaron 98 pacientes, ya que se excluyeron 4 (4%) por fallecimiento, 8 (7%) se retiraron del estudio. Para analizar los resultados, los puntajes de las distintas pruebas neuropsicológicas administradas fueron convertidos a puntuaciones Z (basados en el rendimiento de los sujetos). De esta manera el rendimiento en una prueba se consideró como normal, si el puntaje Z correspondiente era mayor a -1,. si el puntaje estaba entre -1 y -2, fue considerado como disfunción leve y puntajes inferiores a -2 fueron interpretados como deterioro moderado o severo. Resultados: los datos demográficos de la muestra indican que el 62% fueron hombres y 38% mujeres, con edad promedio de 56 años. Se encontraron factores de riesgo como hipertensión arterial (50%) y tabaquismo (37%). La evaluación neuropsicológica inicial demostró que el 42% de los casos tuvieron al menos una alteración en algún dominio cognoscitivo, siendo los más frecuentes: funcionamiento cognoscitivo general, fluidez verbal fonética, reducción en la velocidad de procesamiento de la información y flexibilidad cognoscitiva. En la valoración anual se observó un deterioro en la ejecución de las pruebas que valoran lenguaje en comparación con la valoración inicial (15% vs 23%), percepción (16% vs 22%) y funcionamiento cognoscitivo general (14% vs 18%), siendo estadísticamente significativo (p< 0.001). Conclusiones: las funciones neuropsicológicas más sensibles al deterioro cognoscitivo encontradas en pacientes con EVC fueron lenguaje, percepción y funcionamiento cognitivo general. Como se ha reportado en varios estudios clínicos los cambios cognitivos asociados a enfermedad vascular cerebral son una entidad frecuente, los cuales pueden ser desde cambios mínimos como en las alteraciones vasculares cognitivas sin demencia o llegar a presentar alteraciones vasculares cognitivas con demencia. Se sugiere por lo tanto una adecuada evaluación y diagnóstico para poder establecer medidas de control de los factores de riesgo implicados en el desarrollo de alteraciones cognitivas, para evitar su progresión hacia grados de mayor severidad o demencia vascular.
