Depresión en pacientes con neurocisticercosis.

- Abstract
Antecedentes: existen algunos reportes acerca de la presencia de trastornos depresivos en sujetos que padecen neurocisticercosis, sin embargo sólo se han publicado reportes de casos, estudios retrospectivos o estudios prospectivos con tamaños de muestra muy reducidos y en pacientes que ya han recibido tratamiento antiparasitario. Objetivos: determinar la frecuencia de depresión en pacientes con neurocisticercosis no tratada, así como los factores asociados con el trastorno neuropsiquiátrico. Material y métodos: realizamos un estudio transversal analítico. Mediante un muestreo no probabilístico de casos consecutivos, incluimos sujetos con NCC definida por los criterios de Del Brutto (Neurology, 2000), antes de la administración del tratamiento antiparasitario. Un neuropsiquiatra realizó entrevistas para aplicar un instrumento de diagnóstico psiquiátrico (SCID). El diagnóstico definitivo se obtuvo mediante los criterios del DSM-IV. Se registraron variables demográficas, clínicas, neurobiológicas (a partir de estudios de TAC, RM, LCR). Asimismo aplicamos un instrumento neuropsicológico de rastreo (COGNISTAT) y la escala de calidad de vida de Spitzer. Realizamos estadística descriptiva y un análisis bivariado para explorar factores que podrían estar asociados con la presencia del síndrome psiquiátrico. Resultados: 90 pacientes con NCC no tratada fueron incluidos; tenían una mediana de 36 años de edad, con un rango de 18 a 69 años. 44.4% (n=40) eran mujeres, y 56.6% (n=50) eran varones. La escolaridad fue inesperadamente alta, con una mediana de 9 años, y un rango de 0 a 16 años. El trastorno del estado de ánimo, con síntomas depresivos, se presentó en 22% (n=20), y presentó comorbilidad con ansiedad (13.3%, n=12) y psicosis (4.4%, n=4). Al comparar a los pacientes con y sin depresión, no encontramos diferencias significativas en cuanto a edad, escolaridad, carga parasitaria, características del líquido cefalorraquídeo, presencia de epilepsia o frecuencia de crisis epilépticas, actividad parasitaria, localización de los parásitos, presencia de aracnoiditis, complicaciones vasculares o la presencia de NCC racemosa. Solamente se encontraron diferencias" significativas en cuanto a género y la presencia de otros trastornos mentales, como puede observarse en la tabla 1. El índice de calidad de vida fue significativamente menor en los pacientes con depresión (p <.001). En la tabla 2 se muestra la comparación entre ambos grupos (depresión/no-depresión) con respecto al funcionamiento cognoscitivo. Únicamente la orientación fue significativamente más deficiente en pacientes deprimidos. Conclusiones: la depresión es frecuente en pacientes con neurodsticercosis no tratada, por encima de lo esperado en la población general; es más frecuente en mujeres, y se asocia frecuentemente con la presencia de un trastorno de ansiedad, y en menor grado, con un trastorno psicótico. Nuestro análisis no permitió identificar otros factores clínicos o neurobiológicos asociados con la presencia de depresión. El reconocimiento de estos trastornos permite un tratamiento médico capaz de mejorar la salud mental y la calidad de vida de estos sujetos, la cual es significativamente menor en los pacientes con NCC y depresión.
