Sclerosing Mesenteritis, A Diagnostic Dilemma: Case Report & Literature Review.

- Abstract
Background: Sclerosing mesenteritis (SM) is a fibro-inflammatory condition of the mesentery. This rare condition presents with unspecific symptoms and is often indistinguishable from malignancies. An elderly man with a significant medical history presented with prolonged fever, cough, dyspnoea and weight loss. Laboratory tests were normal apart from raised Erythrocyte Sedimentation Ratio (ESR). Malignancy was suspected requiring and patient subsequently underwent CT imaging & diagnostic laparoscopy. Method: This article is a case report therefore no specific methodology applies. Results: Apart from minor pneumonia, a suspicious 'misty' mesenteric lesion with lymphadenopathy was incidentally discovered on CT. An interval CT scan detected further progression of the lesion with unchanged lymphadenopathy. At diagnostic laparoscopy an area of mesentery had necro-fibrotic features and biopsied. Histopathological analysis of biopsy specimens established the diagnosis. Conclusion: This case highlights diagnostic challenges of SM. Radiological diagnosis from CT is possible however complex patients need biopsies for a reliable diagnosis. Our case also highlights potential of laparoscopy in identifying SM.
