Sistemik skleroz patogenezindeki epitel-mezenkimal geçiş (EMT) yollağı ile ilişkili mikroRNA'ların araştırılması: Yeni bir biyomarker olabilir mi?

- Abstract
Amaç: Sistemik skleroz (SSk) patogenezinin, epitel mezenkimal geçiş [epitelial mesenchimal transition (EMT)] yolağı ile ilişkisini belirlemektir. Yöntem: Punch biyopsi örnekleri, ACR/EULAR (2013) sınıflandırma kriterlerini karşılayan, SSk tanısını yeni alan ve tedavi başlanmamış olan hasta gruplarından alındı. Ayrıca SSk tanısı olmayan sağlıklı kontrollerden de biyopsi alındı. Kyoto Encyclopedia of Genes and Genomes (KEGG), Gene Ontology (GO), Pathway Database (Reactome) ve Epithelial-Mesenchymal Transition Database v2 (dbEMT2) veri tabanları kullanılarak EMT ile ilişkili merkezi işleve sahip gen paneli oluşturuldu. Oluşturulan gen paneli RT-PCR yöntemi kullanılarak RNA izolasyonları gerçekleştirilen 9 hasta örneği ve 4 sağlıklı kontrol örneğinde incelendi. Bu araştırma için yerel etik kuruldan onam alındı. Sonrasında da üniversitemiz Bilimsel Araştırma Proje Birimi'ne başvurularak 2 yıl süreyle "Genel Araştırma Projesi" desteği alındı. Projemizin ilk aşama sonuçları sunulmaktadır. Bulgular: Hastalara ait demografik özellikler Tablo 1'de sunulmuştur. RT-PCR sonuçlarına göre EMT ile ilişkili 6 gende istatistiksel olarak anlamlı değişimler gözlenmiştir (Grafik 1). SSk hastalarından sağlıklı kontrollere oranla AKT1, SMAD4, ERBB2, IGF2, FGFR2, STAT3 genlerinde ekspresyon artışı gözlenmiştir. Ayrıca Raynaud fenomeninin uzun yıllardır olması ve deri bulgularının şiddetine göre gen ekspresyonları korelasyon göstermiştir. Sonuç: SSk'da fibrotik bozukluk hastalığın birçok klinik bulgusunun oluşmasında önemlidir. SSk'daki fibrozis sürecinde EMT yolağına yönelik terapötik veya diagnostik hedef/hedefler henüz tam olarak araştırılmamıştır. Çalışmamızın başlangıç verilerinde genlerde anlamlı ekspresyon artışı saptanmış olup bir sonraki aşamada Microarray yöntemi ile ilişkili miRNA'ların belirlenmesi amaçlanmaktadır.
