- Abstract
The article discusses the authors' views about patient-related medication reconciliation in connection with hospital admissions and discharges, and it mentions the need for medical personnel to create and maintain correct medication lists for every patient. Physician-patient relations and electronic medical records are examined, along with an effort to minimize a patient's out-of-pocket expenses for expensive medications. The goal of improving patients' outcomes is assessed.
