- Abstract
This document is a reply to comments and suggestions made by Dr. Butt and colleagues regarding a paper on blood transfusion in cardiac surgery patients. The authors used the Delphi method to select variables for their analysis and excluded variables that were less relevant to blood transfusion to create a more streamlined prediction score. They did not include tranexamic acid in their initial analysis because it is routinely administered to all patients in their hospital, and they did not include certain variables due to the limitations of their retrospective approach. The authors also discussed their blood conservation techniques during surgery and explained their rationale for certain decisions, such as including emergency cases in their model. They acknowledged the limitations of their study and mentioned plans to improve their variable selection process in future studies. They also addressed the practicality of their modified predictive score within the Chinese context and mentioned plans to include ethnic information in their analysis. The authors acknowledged that their scoring system may slightly overestimate transfusion risk in the high-risk group due to the small sample size, but they maintained that the score is still clinically useful. They mentioned ongoing research and a randomized controlled trial to evaluate the efficacy of their modified system. The authors emphasized the importance of blood management during cardiac surgery and the need for a practical predictive tool for perfusionists.
