Impacto do diagnóstico de sepse durante plantões noturnos e finais de semana na aderência aos guidelines de tratamento: um estudo multicêntrico de coorte.

- Abstract
Objetivo: Avaliar o efeito do diagnóstico de sepse em diferentes períodos do dia (diurno/noturno) e finais de semana na adesão aos pacotes de tratamento e mortalidade hospitalar. Métodos: Estudo observacional, multicêntrico, retrospectivo, incluindo pacientes com sepse (2010 a 2017) em dez hospitais (Rede Amil) de São Paulo. Adesão ao pacote de 3 horas foi avaliada pelo momento de diagnóstico: dia (07:00h às 18:59h) versus noite (19:00h às 06:59h) e dias da semana (7:00h segunda-feira às 18:59h sexta-feira) versus finais de semana (19:00h sexta-feira às 06:59h segunda-feira). O desfecho primário foi adesão ao pacote 3horas e análise multivariada ajustada para idade, sexo, gravidade, local diagnóstico e hospital foi realizada para associar período de diagnóstico com adesão. Resultados: Incluímos 11737 pacientes (8733 sepse/3004 choque séptico). O SOFA foi 4(2-7) e mortalidade 24,7%. Adesão total ao pacote foi maior no período diurno (dia 80,6% vs noite 76,3%, p<0,001), principalmente para lactato (dia 95,6% vs noite 94,4%, p=0,003) e antibióticos (dia 90,2% vs noite 87%, p<0,001). A chance de adesão foi maior no período diurno (OR 1,35 IC95%1,23-1,49, p<0,001). Não houve diferença na adesão nos finais de semana versus semana. A mortalidade hospitalar foi similar para diagnósticos comparando períodos do dia ou da semana. Conclusão: A adesão aos bundles de sepse pode ser influenciada pela hora do diagnóstico, porém não influencia nos desfechos.
