- Abstract
Einleitung: Kopfschmerzen sind ein sehr häufiges Symptom, und der Bildgebung kommt eine entscheidende Rolle beim Ausschluss sekundärer Kopfschmerzursachen zu. Zur klinischen Differenzierung zwischen primären und sekundären/symptomatischen Kopfschmerzen sind eine exakte Anamneseerhebung und klinische Untersuchung wichtige Faktoren. Ziel der vorliegenden Studie ist, Hinweise in Anamnese und klinischer Untersuchung zu identifizieren, die mit einer sekundären Kopfschmerzursache assoziiert sind. Zudem gibt die Studie einen Überblick über die auftretenden Ursachen und Differenzialdiagnosen der sekundären Kopfschmerzformen.Methoden: Es erfolgte eine retrospektive Analyse von 239 Patienten mit einem Alter von ≥18 Jahren und Kopfschmerzen, die eine Computertomographie oder Magnetresonanztomographie mit Darstellung der Sinusvenen erhalten hatten. Die Signifikanz eines Zusammenhangs von Basischarakteristika wie Alter und Geschlecht sowie anamnestischen Angaben (Schmerzintensität, thrombembolisches Risikoprofil) und klinischen Befunden (fokal-neurologisches Defizit, Stauungspapille, Vigilanzminderung) mit einem pathologischen Befund in der Bildgebung wurde mittels χ2-Test (Signifikanz-Level p < 0,05) ermittelt.Ergebnisse: Bei 27 (11,3 %) der untersuchten Patienten konnten mittels Bildgebung behandlungsbedürftige Befunde festgestellt werden. Die häufigsten Befunde waren Zeichen einer intrakraniellen Drucksteigerung (9 Patienten), eine zerebrale Raumforderung (7) und eine Sinusthrombose (3). Es bestand ein signifikanter Zusammenhang zwischen dem Vorliegen eines relevanten pathologischen Befundes in der Bildgebung und einem neurologischen Defizit (p = 0,001) sowie dem Vorliegen einer Stauungspapille (p < 0,001). Die Faktoren Vigilanzminderung, Schmerzintensität, thrombembolisches Risikoprofil sowie Alter und Geschlecht waren nicht signifikant.Schlussfolgerung: Für eine effektive Selektion von Patienten mit Kopfschmerzen ist ein begleitendes neurologisches Defizit ein wichtiger Hinweis auf eine sekundäre Kopfschmerzursache, und insbesondere bei Vorliegen einer Stauungspapille sollte eine Bildgebung durchgeführt werden. Die Qualität der durch den behandelnden Neurologen durchgeführten Anamnese und klinischen Angaben hat damit einen wichtigen Einfluss auf die Differenzierung der Kopfschmerzursachen, wohingegen die durch den Patienten angegebene Schmerzintensität keine wesentlichen Hinweise bieten muss.
