- Abstract
Endometriosis is often neglected, and the delay prior diagnostic and treatment is of 8 years in average in developed countries. This delay in diagnosis is a public health issue as it induces or worsen stress and the feeling of isolation and incomprehension of patients and has a major economic impact on patients and society. Human miRNAs are highly conserved non-coding RNAs composed of 21-25 nucleotides that bind to their complementary messenger RNA (mRNA) thereby regulating the degradation and translation of the target gene. Thanks to the latest new generation sequencing techniques and the use of artificial intelligence algorithms, a saliva test integrating a combination of 109 miRNAs has recently been developed from the ENDOmiRNA cohort for the diagnosis of endometriosis. The performances of this test, subject to an ongoing multicenter validation study, are much higher than those reported so far. The respective sensitivity, specificity, and area under the curve (AUC) for the miRNA signature comprising the 109 miRNAs were 96.7%, 100% and 98.3%. The objective of this article is to place the saliva test in its context and to discuss its potential place in the management algorithm for patients with suspected endometriosis.
