- Abstract
Multiple contractors were employed on a site for a project to install a newly fabricated and refractory lined cross tube which was to be located between two vertical post combustors. The contractor responsible for performing the lift arrived on site without a banksman, so a worker from another contractor stood on scaffolding to perform hand signals for the crane driver to position the cross tube. He ventured onto an unsecured panel on the roof, which unfortunately gave way and resulted in serious injuries to the worker. The main learning points from the incident related to the site operator's management of the interface between various third party contractors, and to guard against the tendency of some contractors to expose themselves to high risks in order to finish a job.
