COLATERALIDAD COMO FACTOR DE BUEN PRONÓSTICO EN PACIENTES CON OCLUSIÓN CAROTÍDEA.

- Abstract
Objetivo: Determinar el impacto del estatus de colateralidad leptomeníngea en el pronóstico funcional a 90 días de los pacientes con enfermedad carotídea sintomática (ECS) por oclusión o estenosis grave (70-99%). Antecedentes: La enfermedad de gran vaso es una de las principales causas de infarto cerebral (IC)1, aproximadamente 15% de todos los infartos cerebrales son causados por oclusión de la arteria carótida (OAC)2. La OAC está asociada a tasas de morbimortalidad elevadas3 y es un reto terapéutico debido a la poca disponibilidad de opciones terapéuticas2. La circulación colateral ha sido reconocida recientemente como un factor determinante en el riesgo de IC y en los patrones de afectación en pacientes con enfermedad carotídea4. Métodos: Estudio transversal retrospectivo; se realizó un subanálisis del protocolo aprobado por comité de ética con oficio folio 84/18 No. CEI/143/18. Se incluyeron pacientes con ECS por OAC y estenosis grave (70-99%) tratados en el Instituto Nacional de Neurología y Neurocirugía, se evaluó el grado de colateralidad en cada paciente de acuerdo a la escala Tan, se definió buen estado de colateralidad puntaje Tan 3-4, llenado >50% en el flujo distal al vaso ocluido. El objetivo primario fue la presencia de excelente pronóstico funcional a 90 días posterior al IC o ataque isquémico transitorio (AIT), escala Rankin 0-1. Los objetivos secundarios fueron la evaluación de recurrencia AIT/IC y muerte. Se realizó regresión logística multivariada con el software STATA versión 17. Resultados: Se identificó un total de 170 pacientes, con edad media de 69 años (+20), 107 (62.9%) fueron hombres. Con oclusión 91 (52.4%) pacientes y con oclusión grave 81 (47.6%). En 55 pacientes (32%; p= 0.089) se observó un excelente pronóstico funcional a 90 días, 15 presentaron recurrencia (8.7%; p=0.631) y 4 fallecieron (2.3%; p= 0.395), no se identificaron diferencias entre ambos grupos. Fue factible evaluar colateralidad en 82 pacientes, 62 (70.45%) tuvieron buen estado de colateralidad y este se asoció a un excelente pronóstico funcional a 90 días en ambos grupos, sin diferencias estadísticas después de ajustar por edad y género, con OR 4.67; (p= 0.015, CI 1.34 - 16.28). Conclusión: En pacientes con OAC, la buena colateralidad se asoció con un pronóstico funcional excelente, sin diferencias entre oclusión y estenosis grave. No se encontraron diferencias estadísticamente significativas entre la tasa de recurrencia o mortalidad entre ambos grupos.
