- Abstract
La epilepsia refractaria es un problema mundial de salud. Se calcula que alrededor de un 30% de los pacientes epilépticos son refractarios al tratamiento médico con dos o más fármacos antiepilépticos. Debido a que la prevalencia mundial de la epilepsia es de cerca del 1%, más de 300,000 personas podrían padecer de epilepsia refractaria, tan solo en nuestro país, Recientemente, nuestro grupo demostró, en un ensayo clínico abierto, que la dapsona (DDS), un fármaco comúnmente empleado para el tratamiento de la lepra y con capacidad para atenuar las convulsiones inducidas por ácido kaínico es seguro y eficaz para reducir el número de crisis hasta en un 70% en promedio, cuando se emplea como terapia adyuvante en el tratamiento de la epilepsia refractaria. Asimismo, se observó que la combinación de DDS y primidona un fármaco antiepiléptlco que se une al sitio de regulación alostérico del receptor GABAA para prolongar el tiempo de apertura del canal de cloro, resultó ser la mejor combinación como tratamiento antí-epiléptico. Para evaluar la capacidad para atenuar las convulsiones inducidas por ácido kaínico (AK) de la DDS en combinación con la primidona, se probaron dos diferentes dosis de DDS y una dosis de primidona, así como su combinación en un modelo de epilepsia inducido por AK. Se utilizaron ratas macho de la cepa Wistarde 200 a250 g de peso, las cuales recibieron por vía i.p. 10 mg/kg de AK y fueron sometidos o no al tratamiento por vía i.p. solo o en combinación de DDS y/o Primidona 30 minutos previos a la administración del AK. Los animales se dividieron en seis grupos experimentales de manera aleatorizada como sigue: grupo control, AK + vehículo; grupo DDS6, AK + DDS a una dosis de 6.25 mg/k; grupo DDS12, AK + DDS a una dosis de 12.5 mg/k; grupo P30, primidona a una dosis de 30 mg/k; grupo DDS6+P30, AK + DDS a una dosis de 6.25 mg/k + primidona a una dosis de 30 mg/k y grupo DDS12+P30, AK + DDS a una dosis de 12.5 mg/k + Primidona a una dosis de 30 mg/k, se evaluó la frecuencia de crisis límbicas ("sacudidas de perro mojado") durante 2 hs y la presencia de convulsiones tónico-clónico generalizadas por medio de una cámara de video. Los resultados muestran que el tratamiento con DDS y P30 reducen el número de crisis límbicas en 70% para el grupo DDS6,90% en el DDS12 y 80% en el grupo P30. Los grupos tratados con la combinación de los fármacos, observamos una reducción de 94% de las crisis en el grupo tratados con DDS6 y P30 y en los animales tratados con DDS12 y P30 la reducción fue del 50%. Finalmente, sólo la combinación de dosis altas de DDS12 y P30 produjo una inhibición total de las convulsiones tónico-clónico generalizadas (figural). Con estos resultados podemos concluir que la combinación de DDS y P es efectiva para atenuar tanto las crisis límbicas como las convulsiones tónico-clónico generalizadas en el modelo de epilepsia inducida por AK en la rata.
