SENSIBILIZZAZIONE E DERMATITI DA CONTATTO NEI LAVORATORI DELLA SANITÀ: DATI DI PREVALENZA NEL NORD-EST ITALIA.

- Abstract
Background: il lavoro in sanità è spesso associato al rischio di sviluppo di patologie cutanee di origine occupazionale. I lavoratori sono esposti ad agenti quali disinfettanti, saponi, detergenti, lattice e inoltre si richiede loro il lavaggio, frequente e accurato, delle mani. Obiettivi: investigare le associazioni tra le sensibilizzazioni - rilevate al patch test - e il lavoro in ambito sanitario, in una popolazione di pazienti affetti da dermatiti da contatto in varie unità di Dermatologia o Medicina del Lavoro nel nord-est Italia. Pazienti e metodi: sono stati sottoposti a patch test 14464 pazienti (67.6% donne, 32.4% uomini) con sospetta dermatite allergica. Le associazioni tra i risultati del patch test e le professioni sono state studiate con la regressione logistica multivariata. Risultati: i lavoratori della sanità rappresentavano il 6.3% della popolazione studiata (906). La presenza di dermatite a mani e avambracci è risultata significativamente maggiore in questo gruppo rispetto agli impiegati, considerati come categoria di riferimento (donne: RR 2,63, IC 95% 2,13 - 3,24; uomini: RR 2,33, IC% 1,6 - 3,39), rispetto alle altre sedi. Nei soggetti affetti da dermatite limitata alle mani e avambracci, il Thimerosal è stato l;unico aptene ad aver presentato un aumentato rischio nei lavoratori della sanità (OR 2,31, IC 95% 1,49 - 3,6, dati corretti per sesso ed età). Conclusioni: Il nostro studio fa emergere un;associazione tra lavoro in sanità e sensibilizzazione a Thimerosal. Tale aptene è stato sempre associato a dermatiti extraprofessionali e la sua rilevanza è stata discussa in numerosi studi. La nostra indagine, effettuata sul più grosso data base italiano di dermatiti da contatto (a nostra conoscenza) suggerisce un ruolo di quest;aptene presente come conservante in numerosi prodotti e rende necessario valutare più attentamente la possibile rilevanza di questa sensibilizzazione in operatori sanitari esposti a disinfettanti.
