RESPOSTAS CARDIOVASCULARES DE OCTOGENÁRIA HIPERTENSA AO EXERCÍCIO AERÓBIO E ISOMÉTRICO DE PEQUENA E GRANDE MASSA: UM ESTUDO PILOTO.

- Abstract
Introdução: O processo de envelhecimento, principalmente aliado ao estilo de vida sedentário, é um fator atuante na prevalência da hipertensão arterial (HA). O exercício físico pode exercer papel adjuvante na prevenção e tratamento da HA. O exercício aeróbio pode influenciar diferentes respostas cardiovasculares quando comparado com o treinamento de força (TF). Atualmente o TF isométrico de preensão palmar (handgrip) apresenta importante relevância clínica no controle da HA. No entanto, há um hiato na literatura sobre quais seriam os resultados do exercício isométrico utilizando uma grande massa muscular envolvida, como no equipamento Leg Press. Contudo, a proposta do estudo foi analisar os parâmetros cardiovasculares induzidos pelo exercício aeróbio e isométrico com pequena e grande massa muscular. Metodologia: Participante Idosa de 89 anos, 67 quilos, estatura 1,51 metro, índice de massa corporal 29,6 kg/m², hipertensa medicada há 42 anos, atualmente com Losartana 25 mg/dia e Levotiroxina 25 mg/dia, regularmente ativa há dois anos. Esse trabalho foi aprovado pelo comitê de ética em pesquisa com seres humanos na Universidade Católica de Brasília (parecer nº 1.269.917/2015). Testes de Contração Voluntária Isométrica Máxima (CVIM) O teste de contração voluntária isométrica máxima (CVIM) foi realizado em um dinamômetro hidráulico de preensão palmar (Jamar, IL, USA) e em um equipamento Leg press 45º utilizando uma célula de carga Din Pro (Cefise, Campinas, SP). Sessões do Exercício Isométrico A partir da obtenção das cargas de CVIM, as sessões isométricas foram realizadas com 30% da CVIM, divididas em 8 séries com 1 minuto de contração e 2 minutos de pausa. As sessões foram randomizadas e aleatórias, com intervalo mínimo de 48h. Sessão de Exercício Aeróbio A frequência cardíaca (FC) adotada na esteira foi 70% da fórmula de predição da FC máxima proposta por Tanaka: 208 - (0,7 * idade) ≈ 100 batimentos por minuto. Também foram realizadas 8 séries com 1 minuto de execução e 2 minutos de pausa. Sessão controle Na sessão controle a idosa compareceu ao laboratório, posicionou-se no aparelho sem realizar as contrações e os parâmetros cardiovasculares foram aferidos nos mesmos intervalos das sessões de exercício. Medida da Pressão Arterial e Frequência cardíaca A pressão arterial sistólica (PAS), diastólica (PAD) e FC foram aferidas por monitor oscilométrico Microlife 3AC1-1 nos tempos repouso, final da sessão e 5', 10', 15' e 30' de recuperação. A pressão arterial média (PAM) foi calculada por (PAS - PAD) / 3 + PAD. O duplo produto (DP) foi obtido através da multiplicação da FC pela PAS. Resultados e Discussão: A resposta pressórica da sessão usando o handgrip (HG), com pequena massa muscular envolvida, foi mais evidente do que a sessão usando o exercício isométrico LP (grande massa muscular). Esta resposta pode ser decorrente da contração de músculos sinergistas⁵ e da pequena área vascular que percorre os antebraços em relação aos quadríceps e isquiotibiais, resultando em um acúmulo ao invés da distribuição de metabólitos, como K+ e lactato, sinalizadores aos quimioceptores que conduzem o feedback ao centro cardiovascular. O HG apresentou menor pico de pressão de pulso (PP) quando comparado ao LP e esteira, além de gerar uma queda considerável na PP, chegando a -20 mmHg em relação ao repouso no trigésimo minuto de recuperação. O duplo produto (DP) envolve inotropismo e cronotropismo, apresentando uma forte correlação com o consumo de oxigênio pelo miocárdio. Exercícios predominantemente dinâmicos envolvendo grande massa tendem a demandar um aumento do débito cardíaco, pequeno acréscimo relativo da PAS e estabilidade da PAD, exercendo principalmente uma carga de volume no coração. Já os exercícios isométricos direcionam um aumento tanto da PAS quanto PAD com pequena elevação relativa do débito cardíaco, produzindo uma carga cardíaca principalmente pressórica⁴. Esses mecanismos podem ser inferidos nas respostas de DP e PAM. Conclusão: O equipamento handgrip possui ótima aplicabilidade, demanda pouca energia³, gera baixa carga de volume cardíaco e elicitou resposta hipotensora após o exercício¹. Essas características alimentam o viés da preensão palmar como uma intervenção não medicamentosa segura no controle da HA. O exercício aeróbio, forma de treinamento mais estudada e prescrita para o controle da HA, exigiu uma resposta cardiovascular mais expressiva do que os exercícios isométricos, principalmente em relação à FC e DP. As duas modalidades de exercício produzem efeitos favoráveis na saúde, como o aumento da força e resistência muscular e do VO2 máximo. Portanto, o exercício predominantemente aeróbio e o treinamento de força isométrico, ambos inseridos numa periodização e prescrição devidamente adaptada, podem gerar benefícios a idosos e muito idosos hipertensos.
