Proximal subclavian ulcerative atheromatous lesion can present as vertebrobasillar TIA from atherothromboembolic phenomena. Because it lacks hemodynamic significance, prosthetic graft bypass may undergo thrombosis. Endarterectomy and transposition of the subclavian artery, which has had good results, is the procedure of choice in such cases.
