- Abstract
Introducción. La nefropatía diabética (ND) es un problema de salud pública cada vez mayor en todo el mundo cuya incidencia y prevalencia van en aumento y que la mayoría de las veces se detecta en estadios tardíos. Se sabe que el incremento del estrés oxidativo contribuye de forma importante en la patogenia multisistémica de la diabetes mellitus. Objetivo. Determinar el estado de oxidación en pacientes con nefropatía diabética a punto de iniciar su tratamiento con diálisis peritoneal y compararlos con un grupo de pacientes con nefropatías no diabéticos y un grupo de control (sanos). Materiales y métodos. Se hizo un estudio transversal, observacional, analítico y comparativo, con un total de 118 sujetos (57,6 % mujeres y 42,3 % hombres). El 53,3 % tenía diagnóstico de ND en estadio 5, el 12,7 % no era diabético pero tenían nefropatías en estadio 5. En las muestras sanguíneas se midieron los antioxidantes VC, SOD, y GSH y los oxidantes NOx, MDA, AGE y AOPPs con espectrofotometría. Resultados. No hubo diferencias en el VC (p=0,2594) ni el SOD (p=0,7470). La actividad del GSH fue mayor en los sanos (p<0,0001) que en los ND. Los biomarcadores de oxidación como NOx y MDA no registraron diferencias p=0,5924 y p=0,5134, respectivamente. Los AGE presentaron diferencias (p=0,0043), con mayor concentración en los ND que en las personas sanas. También hubo cambios (p<0,0001) en la AOPP, la cual fue mayor en la ND. Conclusiones. Los biomarcadores antioxidantes se encontraron disminuidos en los pacientes con nefropatía diabética y no diabética y los oxidativos estaban incrementados.
