The article discusses documentation mistakes that U.S. podiatrists should avoid. Topics discussed include the importance of demonstrating follow-up in the patients' charts or medical records, the threat of malpractice in relation to practice owner's signing of medical record instead of the provider, and common mistakes in medical records that should be avoided including incorrect history, infrequent vital signs recording and the tendency to copy and paste inaccurate information.
