- Abstract
As cólicas abdominais são comuns nos equinos, sendo especialmente desafiadoras nos neonatos. Entre os pacientes com menos de 14 dias de vida, as causas mais comuns dos desconfortos estão ligadas a afecções de resolução clínica, como compactação/retenção de mecônio, enterocolites, íleo adinâmico e cólicas transitórias de origem desconhecida. A decisão por uma intervenção cirúrgica pode ser especialmente difícil nos neonatos, pela inespecificidade de sinais, presença de doenças concomitantes, maior probabilidade de formação de aderências, resultado a longo prazo desconhecido e fatores econômicos, ainda que estudos demonstrem taxas de sobrevida próximas em relação a animais tratados clínica ou cirurgicamente. Assim como em cavalos adultos, a decisão pela cirurgia tem que ser tomada o quanto antes e deve ser levada em consideração toda informação obtida com exames complementares e, principalmente, evolução clínica do paciente. Os achados cirúrgicos mais comuns nos neonatos equinos são: obstrução estrangulativa do intestino delgado, atresia intestinal e uroperitônio, enquanto obstruções não estrangulantes de intestino grosso são menos comuns. Ainda, contém estruturas menores em relação aos animais adultos e têm mesentério e vasos sanguíneos mais sensíveis à manipulação. Chegou ao hospital veterinário um equino, com 12 horas de vida e histórico de ter demorado para levantar-se após o parto, não ter liberado mecônio e ter distensão abdominal bilateral. Apresentava taquicardia, taquipneia, hipomotilidade e mucosas hiperêmicas. Após administração de enema comercial, o animal defecou, porém, continuou apresentando desconforto e não respondendo à analgesia. Optou-se por encaminhá-lo à celiotomia exploratória, que revelou destroflexão da flexura pélvica e compactação em cólon menor. A terapia pós-cirúrgica incluiu antibioticoterapia, anti-inflamatórios, protetores gástricos e heparina SC. No segundo dia pós-cirúrgico, apresentou estertor pulmonar bilateral e foi tratado com penicilina, clembuterol, acetilcisteína e nebulização com solução fisiológica e gentamicina. No quinto dia pós-cirúrgico, apresentou quadro de diarreia, tratado e resolvido com probiótico e suplemento aminoácido comercial. Teve alta hospitalar após 21 dias, sem ter apresentado outro quadro de desconforto. Apesar da literatura descrever afecções de intestino grosso como menos comuns, o presente relato apresentou destroflexão da flexura pélvica como causa da indicação cirúrgica. Além disso, são relatadas maiores incidências de aderências intestinais e dificuldade de manuseio das alças intestinais em animais neonatos. Felizmente, não houve indícios da formação de aderências nesse pósoperatório. A literatura também descreve a dificuldade na tomada de decisão por cirurgia. Neste caso, graças à observação cuidadosa do paciente, foi possível realizar a intervenção rápida, contribuindo para melhor evolução clínica e sobrevida do animal.
